The crash happened at the intersection of Albany Highway and Nicholson Road in Beckenham, with multiple lanes blocked.
Traffic is heavy on approach and drivers have been advised to seek an alternative route where possible.
Traffic remains busy in Whiteman despite an earlier serious crash at the intersection of Marshall and Altone roads being cleared.
The incident caused significant disruptions during the morning peak, with congestion continuing to linger as traffic slowly returns to normal.
A burst water main is affecting traffic in Stirling, with the left-turn lane on the Karrinyup Road northbound entry from San Remo Boulevard blocked by Water Corporation crews.
A reduced speed limit of 40km/h is in place and motorists have been urged to take care.
On Mitchell Freeway, a vehicle breakdown southbound before Powis Street in Osborne Park has left a vehicle stranded in the left emergency lane.
Traffic is flowing through the area, but drivers are being reminded to stay alert.
Another breakdown remains on Mitchell Freeway southbound after Warwick Road in Warwick, where a vehicle is stopped in the right emergency lane.
No major delays have been reported, however motorists have been urged to proceed with caution.
Motorists have also been warned about a tyre in the right lane on the Kwinana Freeway southbound entry from Beeliar Drive in Atwell.
Earlier incidents at South Street in Bateman and on Graham Farmer Freeway near the Northbridge Tunnel have been cleared.
